Important: All minors are to sign this agreement at the bottom and return it before the ride.

PROM CONDUCT AGREEMENT

1. A group leader will be chosen and the name given to the driver. 

2. A parent's name and the phone number of a parent who will be home
    will be given to the driver for emergencies.

3. The driver will verify this number.

4.  No drugs, smoking, or alcohol.

5.  The sun roof will be locked closed.

 6. The solid divider and glass divider will be locked open.

 7. If anything is broken or damaged there is a $100 minimum charge.

 8. If anyone gets sick in the car there is a $65 clean up charge.

 9. If anyone in the car is a minor everyone in the group must sign.

10. This form is to be printed, signed and faxed prior to the ride or handed
      the chauffeur before the ride.
