DISCLAIMER

We, the undersigned, do hereby agree that Uptown Limousine Service and it's employees will not supply or serve any type of alcoholic beverages in the limousine hired for this evening's prom.

If any type of alcohol is brought into the limousine, trunk or passenger's compartment of the limousine, Uptown Limousine Service will not be responsible for the actions of its passengers.

In the event alcohol is found in the vehicle by Uptown limousine Service or it's employees, the limousine ride will cease and be terminated immediately, with no refunds given.

Uptown Limousine Service will also cease to be obligated by any conditions and/or agreements or implied conditions and/or agreements concerning the reservation for the limousine ride and will be free of any present and future claims.
